To simplify your intake at Lakeview Counseling, we’ve made our initial paperwork available online. Before you come in for your first session, please fill out and sign the paperwork below.

Completing the paperwork ahead of time lets you save time in our office, as well as look up important dates or specific details in your records.

Working with these documents

These forms may be opened and printed in your browser and filled out and signed by hand, or filled out and digitally signed on your computer using the free Adobe Acrobat Reader program. As a health care provider, we abide by the Health Insurance Portability and Accountability Act (HIPAA) to protect your privacy.
