To simplify your intake at Lakeview Counseling, we’ve made our initial paperwork available online. Before you come in for your first session, please fill out, sign and submit the paperwork below.
Completing the paperwork ahead of time lets you save time in our office, as well as look up important dates or specific details in your records.
Working with these documents
These forms can be securely completed and electronically signed and submitted in your browser, or downloaded, filled out and signed. As a health care provider, we abide by the Health Insurance Portability and Accountability Act (HIPAA) to protect your privacy. NOTE: Please fill out either the adult or minor forms, and both the release of information and treatment and billing authorization forms.
